Welcome to Bluesky Wellness Clinic (“Bluesky,” “Clinic,” “we,” “our,” or “us”). These Terms of Service (“Terms”) govern your use of our website, appointment scheduling tools, text messaging and email communications, and services provided by Bluesky Wellness Clinic.
By accessing our website, scheduling an appointment, submitting information, communicating with us electronically, or receiving services from us, you agree to these Terms.
1. Services We Provide
Bluesky Wellness Clinic is a California medical clinic that offers services including:

Immigration medical examinations, including exams related to Form I-693

Vaccination review and administration, as medically appropriate and available

Access to laboratory testing and related diagnostic services, as needed

Related medical and administrative services connected to immigration physicals
Our clinic provides medical services only. We do not provide legal advice, immigration strategy, or representation before USCIS or any other government agency.
2. No Legal Advice or Immigration Guarantee
Bluesky Wellness Clinic is a medical provider, not a law firm. Information on this website or provided by our staff is for general informational and medical service purposes only and is not legal advice.
Receiving an immigration physical, vaccinations, lab testing, or completed medical forms from our Clinic does not guarantee:

approval of any immigration application or petition,
-
acceptance of any filing by USCIS,
-
waiver eligibility,
-
avoidance of delays or requests for additional evidence, or
-
any specific immigration outcome.
Final decisions are made by the appropriate government agency, not by our Clinic.
3. Patient Responsibilities
You agree to provide complete, accurate, and truthful information to the Clinic. You are responsible for:

bringing valid identification,
-
bringing vaccination records and prior medical records if available,
-
completing forms honestly and fully,
-
following clinical instructions,
-
attending required follow-up appointments if needed,
-
reviewing any unsealed patient copy of documents for obvious demographic errors when provided, and
-
keeping copies of records for your own files.
Incomplete, inaccurate, or misleading information may delay or prevent completion of your exam or paperwork.
4. Immigration Physicals and Medical Documentation
Immigration medical exams must be completed in accordance with applicable medical and administrative requirements. The provider may require additional history, examination steps, vaccinations, laboratory testing, treatment documentation, or follow-up before the Clinic can complete the required medical paperwork.
We reserve the right to delay, decline, or refrain from completing or releasing medical forms if:

required information is missing,
-
required testing or vaccination documentation is incomplete,
-
additional clinical review or follow-up is needed,
-
a patient does not cooperate with exam requirements,
-
fraud, falsification, or document tampering is suspected, or
-
completion would be inconsistent with legal, regulatory, or professional requirements.

5. Vaccinations and Lab Testing
As part of an immigration physical or related medical service, Bluesky Wellness Clinic may provide vaccinations and access to laboratory testing as needed.
You understand and agree that:

not all vaccines or tests are included in the base exam fee unless expressly stated,
-
additional charges may apply for vaccines, lab work, repeat testing, or outside services,
-
some laboratory services may be performed by third-party labs,
-
test turnaround times may vary and are not guaranteed,
-
further follow-up may be needed depending on results.

6. Appointments, Cancellations, No-Shows, and Late Arrivals
Appointments are subject to availability.
Cancellation Policy: Appointments canceled with less than [24 hours] notice may be subject to a cancellation fee.
No-Show Policy: Failure to appear for an appointment may result in a no-show fee and may require prepayment before rescheduling.
Late Arrivals: Patients who arrive late may need to reschedule depending on provider availability.
We reserve the right to reschedule appointments due to provider availability, emergencies, staffing issues, equipment issues, or other operational needs.
7. Fees, Payment, and Refunds
Fees for services may include charges for:

office visit or exam,
-
vaccinations,
-
laboratory testing,
-
administrative processing,
-
form completion or reissuance,
-
follow-up visits,
-
other medically necessary services.
Unless otherwise stated in writing:

payment is due at or before the time of service,
-
quoted fees may not include additional services that become necessary after the initial visit,
-
charges for professional services already rendered are non-refundable,
-
charges for vaccines administered, lab work ordered, and forms prepared are generally non-refundable.
Refunds are not provided for delays caused by missing records, incomplete history, failure to complete follow-up steps, government processing delays, or immigration decisions.
If a correction is required because of a Clinic clerical error, we will address it appropriately. If rework is required because of incorrect patient information, missing documents, opened sealed packets, expired records, missed deadlines, or changed requirements, additional fees may apply.
8. Text Messaging, Calls, and Email Communications
By providing your phone number and email address, you consent to receive communications from Bluesky Wellness Clinic related to your care and appointments, including:

appointment reminders,
-
scheduling communications,
-
intake requests,
-
follow-up instructions,
-
requests for records or vaccine history,
-
billing and payment communications,
-
notifications related to lab testing or needed next steps.
These communications may be sent by phone call, voicemail, email, or text message (SMS).
You understand and agree that:

standard message and data rates may apply,
-
text messages and standard email may not always be fully secure,
-
communications may be seen by others who access your phone, email, or voicemail,
-
you are responsible for providing updated contact information.
You may opt out of non-essential text messaging by notifying us, but doing so may affect our ability to communicate efficiently about your care.
9. Marketing Communications
If Bluesky Wellness Clinic sends promotional or marketing messages, we will do so only in accordance with applicable law and your communication preferences. Consent to receive marketing messages is not required to receive medical care.
You may opt out of marketing communications at any time by following the opt-out instructions in the message or contacting the Clinic directly.

Bluesky Wellness Clinic is not responsible for delays, outages, errors, or policies of third-party providers except as required by law.
